胫腓骨中下段开放性骨折100例治疗分析

目的：探讨胫腓骨中下段开放性骨折的治疗方法及临床疗效。方法：回顾性分析自己10余年所收治的100例胫腓骨中下段开放性骨折的临床资料，进行总结，并随访1—10年。结果：骨折临床愈合、对位对线满意、软组织一期愈合、伤肢功能基本恢复68例，占68％；伤口感染11例，占11％；骨不连或延迟愈合7例，占7％；畸形愈合或钢板断裂4例，占4％；创伤性关节炎、踝关节不同程度僵硬者10例，占10％。结论：胫腓骨中下段开放性骨折的治疗以支架外固定法为首选，钢板螺钉内固定为次，而跟骨牵引配合夹板或“U”型石膏外固定又次之，交锁髓内钉内固定则为相对禁忌。

Treatment and analysis of 100 cases with open fracture in middle and distal segment of tibiofibula

Objective To explore the treatment method and clinical effect of open fracture in middle and distal segment of tibiofibula.MethodThe clinical data of 100 cases with open fracture in middle and distal segment of tibiofibula treated by the author in more than 10 years was analyzed retrospectively,and followed up for 1~10 years.ResultsClinical healing of fracture,satisfactory position and line,primary healing of soft tissue,and basical function restore of wound extremity were found in 68 cases,and accounted for 68%;the wound infection in 11 and accounted for 11%;bone discontinuation or delayed heals in 7(7%);abnormal heals or steel plate broke in 4(4%);traumatic arthritis,stiffness of ankle joint varying different degree in 10(10%).ConclusionThe efficacy of open fracture in middle and distal segment of tibiofibula from excellent to bad is support outside the fixation which is the first choice,internal fixes of steel plate bolt,calcaneus tows coordinating splint or "U" outside the gypsum fixing,while internal fixation of interlocking bone nail is the relative ontraindication.
