经尿道前列腺汽化电切术中应用膀胱穿刺造瘘的体会

目的总结膀胱穿刺造瘘在经尿道前列腺汽化电切术（TUVP）治疗前列腺增生症的应用经验。方法采用单纯汽化电极和汽化切割电极配合膀胱穿刺造瘘施行TUVP300例，为造瘘组，未采取膀胱穿刺造瘘行TUVP的120例作为对照组，对两组术中及术后情况进行比较。结果造瘘组平均手术时间为（58．6±9．4）分钟，对照组为（68．2±11．3）min，两组差异有显著性（P〈0．05）；造瘘组电切综合征（TURS）发生率为2．0％，低于对照组（7．5％）（P〈0．05）。结论在TUVP术中应用膀胱穿刺造瘘，操作简单，创伤小，能降低膀胱内压，增加灌洗液流量，使视野更清晰，同时减少灌洗液的吸收，避免膀胱逼尿肌损害，手术时间减少，降低TURS等并发症的发生率，效果确切。

Experience of bladder puncture in transurethral electrovaporization of prostate

Objective To conclude the experience of the bladder puncture in transurethral electro vaporization of prostate （TUVP）. Methods 300 cases undergone TUVP combined with bladder puncture and 120 cases undergone TUVP without bladder puncture were retrospectively analyzed. Results To compare with operation without bladder puncture, the operation time of operation with bladder puncture was short. The incidence of TURS of operation with bladder puncture was decreased. Conclusion The application of bladder puncture in TUVP can decompress of the bladder, increase perfusion volume and decrease its absorption, make out a clear operating field and reduce the complications such as transurethral resection syndrome.
