桂芪葶百汤配合胸腔灌注化疗治疗肺癌胸水的临床观察

目的 :观察桂芪葶百汤配合胸腔灌注化疗治疗肺癌胸水肺气虚和气阴两虚证的疗效。方法 :选取肺癌胸水肺气虚和气阴两虚证住院患者 5 0例 ,分层后随机分为桂芪葶百汤配合胸腔灌注化疗组 (治疗组 )和单纯胸腔灌注化疗组 (对照组 )各 2 5例 ,从胸水缓解、原发灶变化、生活质量变化、毒副反应等方面进行临床观察。结果 :对肺气虚和气阴两虚证的疗效 ,治疗组优于对照组 (P <0 0 5 ) ;对原发灶的有效率 ,两组比较无显著性差异 (P >0 0 5 ) ;生活质量 (卡氏评分 ) ,治疗组优于对照组 (P <0 0 5 ) ;减轻消化道反应 ,治疗组优于对照组 (P <0 0 5 ) ;肺气虚和气阴两虚证的证型疗效比较 ,无显著性差异 (P >0 0 5 )。结论 :桂芪葶百汤配合胸腔灌注化疗治疗肺癌胸水肺气虚和气阴两虚证疗效明显 ,其作用机制在于两者有协同作用 ,其综合疗效明显优于单纯胸腔灌注化疗。

Clinical Observation on Treatment of Pleural Effusion secondary to Lung Cancer with Guiqitingbai Decoction combined with Chemotherapy

Objective :To observe the curative effects of Guiqitingbai decoction combined with the chemotherapy through the perfusion of the chest on the pleural effusion secondary to lung cancer with the syndrome of Qi deficiency of the lung(SQDL) and the syndrome of both Qi and Yin deficiency of the lung(SQYDL). Methods :50 patients with pleural effusion secondary to lung cancer were secected,and divided randomly into the treatment group(treated with the Guiqitingbai decoction combined with chemotherapy through the perfusion of the chest)and the control group(treated with single chemotherapy through the perfusion of the chest)after stratification,with each group having 25 cases.The clinical curative effects were observed and compared. Results :Based on the clinical observation on the effects of SQDL and SQYDL,the treatment group had a better curative effect than the control group( P <0 05%).There was no difference between the treatment group and the control group in the effective rate of curing the primary focus of pleural effusion secondary to lung cancer ( P >0 05).Evaluated by the scoring method of karnofsky.The quality of life of the treatment group was better than that of the control group( P <0 05).Alleviation of the toxicity and side effects of chemotherapy on the digestive,the treatment group was better than that of the control group( P <0 05).There was no significant difference in the curative effects between SQDL and SQYDL( P >0 05). Conclusion :The therapy of the combination of Quiqitingbai decoction and chemotherapy through the perfusion of the chest has a satisfactory curative effect on the pleural effusion secondary to lung cancer with SQDL and SQYDL.Its mechanism is that both therapies have a synergistic effect and the comprehensive effect is better than that of single chemotherapy through the perfusion of the chest.
