脑瘤病人胎免疫组织细胞移植与肿瘤相关抗原

摘    要:目的:探讨肿瘤相关抗原在脑瘤病人胎免疫组织细胞移植后变化特点及疗效评估。方法:采用放射免疫测定法对42例接受胎胸腺组织、肝脾细胞移植的脑瘤病人作血清甲胎蛋白(AFP)、癌胚抗原(CEA)和铁蛋白(FT)等肿瘤相关抗原测定。采用荧光抗体技术检测抗胎胸腺抗体,严密观察病人的临床症状及体征。将亚恶性脑瘤(包括良性脑瘤)与恶性脑瘤相比较,并将病人移植前、移植后的测定值作自身对照统计学处理。结果:AFP未呈现显著性差异(P>0.05);而CEA和FT则呈现极显著性差异(P<0.01),肿瘤相关抗原水平的测定,反应了不同脑瘤的特点,有助于脑瘤性质的鉴别和预后的评估。移植后病人经抗胎胸腺抗体检测,结果阴性;临床观察也未见任何免疫排斥反应或移植物抗宿主反应(?)象。结论:本组病人移植后临床症状不断改善,肿瘤相关抗原明显降低,表明移植治疗效果是满意的。

Abstract:To survey the tumor associated antigen in patients with brain tumors in whom transplanted with fetal immune tissue and cells, and to estimate its clinical significance. Method: The alpha-fetoprotein ( AFP ) , carcinoembryonic antigen (CEA) and ferritin (FT) were measured by radioimmunoassay in 42 patients with brain tumors in whom transplanted with fetal thymic tissue, liver and spleen cells. With flourescent antibody technique the antifetal thymic antibody in the patients was tested, and the clinical symptoms of the patients were observed. Comparison was made between submalignant brain tumors (including benign brain tumors) and malignant brain tumors after transplantation, and also between pre-and-post transplantation. Result: AFP was not significantly different ( P > 0.05), but CEA and FT were significantly different (P < 0.01) before and after transplantation. This re- sult suggested that the tumor associated antigen level determination could be considered as an ad-junctive diagnostic criterion for differentiation of malignancy of brain tumors and should helpful for the predicting prognosis. It showed negative for the antifetal thymus antibody test in patients after transplantion. The immune rejection and graft antihost reaction were not found in any patient after transplantation.Conclusion:The clinical presentation of the patients were improved and the tumor associated antigen was decreased after transplantion. So the therapeutic effect was satisfactory.
