主动脉弓瘤外科治疗三例

目的总结３例主动脉弓瘤的外科治疗经验，探讨主动脉弓瘤手术并发症的发生及处理。方法１例先天性主动脉弓瘤，用未间断脑血流完成人造血管替代术；１例主动脉弓囊性动脉瘤在深低温停循环下行补片修补术；另１例行升主动脉加主动脉全弓置换术。结果１例囊性动脉瘤主动脉弓有广泛粥样硬化的患者发生脑梗塞，３例患者均痊愈出院，随访３～１２月效果良好。结论中枢神经系统的损害是主动脉弓瘤手术治疗的严重并发症，对粥样硬化所致的主动脉弓瘤，需采取更进一步的措施防止脑栓塞的发生。

Surgical Treatment of Aortic Arch Aneurysm: A Report of 3 Cases

Objective The resection and prothetic replacement of aortic arch aneurysm(AAA) has been considered a formidable procedure with serious complication. This report is to summerize the experience of surgical treatment of AAA. Methods One patient with congenital AAA was operated without using circulatory arrest technique. Under hypothermic circulatory arrest, angioplasty with a patch was performed in the second case of saccular aneurysm of aortic arch, and ascending aorta and total aortic arch replacement was performed in the third case. Results Cerebral embolization occurred in 1 patient with extensive aortic atheroma. All patients had been alive and well 3 to 12 months following surgery. Conclusion Cerebral injury is a serious complication in the operation of AAA. For patient with atheromatous disease, additional methods to prevent perioperative embolic stroke are needed.
