外伤、胃癌、肝硬化脾切除术对术后体温的影响——附81例临床分析

通过对三组81例外伤、胃癌、肝硬化脾切除与对照性非脾切除溃疡穿孔、胃癌、肝硬化手术病人术后动态体温观察及术前术后多项指标比较,结果表明:外伤、胃癌脾切除后体温与对照组相比虽有不同,但无动态性统计学意义。肝硬化切脾术后体温持续地显著高于非切脾对照组。研究中发现,肝萎缩严重者,切脾后发烧重。因此,我们认为:肝硬化是脾切除后发烧的原因。肝病重、发烧重。非肝病脾切除并不明显影响术后体温的恢复。

Effect of splenectomy on postoperative temprature of trauma,stomach cancer,liver cirrhosis patients with splenectomy

The risk of infections after splenectomy is well established.However, there are no adequate tests for evaluating the specific spleen-related functions and pathogen of postoperative fever after splenectomy.We studied three groups splenectomy patients,splenectomy on trauma,have stomach cancer and liver cirrhosis patients with their controls.Our results show that the postoperative temperatures of trauma splenectomy and total gastrectomy with splenectomy are not persistently higher than that of their controls,but postoperative temperature of gastroeso- phageal devascularization with splenectomy are higher than that of patients without splenectomy.We find that the more liver atrophy, the higher postoperative fever is in liver cirrhosis patients.It is suggested that liver cirrhosis is the cause of postoperative fever after splenectomy, because liver is first fender to intestinal tract.We think that endotoxemia from the intestinal tract maybe the pathogen of postopera tire fever after the splenectomy.
