实时三维超声心动图评价2型糖尿病合并心力衰竭患者左心室功能及同步性

摘    要:目的采用实时三维超声心动图(RT-3DE)评价2型糖尿病合并心力衰竭患者左心室功能及同步性。方法选择18例2型糖尿病合并心力衰竭患者(观察组)和18名健康志愿者(正常组),应用RT-3DE测定左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、左心室射血分数(LVEF)、左心室心肌质量(LVM)、左心室心肌质量指数(LVMI)以及左心室容量-时间曲线各参数Tmsv16-SD、Tmsv16-Dif、Tmsv16-SD/R-R(%)、Tmsv16-Dif/R-R(%),并对两组各参数进行统计学比较。结果与正常组比较,观察组LVEDV、LVESV、LVM、LVMI、左心室容量-时间曲线参数Tmsv16-SD、Tmsv16-Dif、Tmsv16-SD/R-R(%)、Tmsv16-Dif/R-R(%)显著升高,而LVEF明显降低(P<0.05)。结论 RT-3DE可反映2型糖尿病合并心力衰竭患者左心室功能异常和收缩不同步,为临床诊断提供可靠依据。

Abstract:Objective To evaluate left ventricular function and synchronism with real-time three-dimensional echocardiography (RT-3DE) in type 2 diabetes with heart failure patients. Methods Eighteen type 2 diabetes patients with heart failure (observation group) and 18 health adults (normal group) were enrolled. Left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), left ventricular ejection fraction (LVEF), left ventricular mass (LVM), left ventricular mass index (LVMI) and left ventricular capacity-time curve parameters including Tmsv16-SD, Tmsv16-Dif, Tmsv16-SD/R-R(%) and Tmsv16-Dif/R-R(%) were measured with RT-3DE and compared statistically. Results Compared with the normal group, LVEDV, LVESV, LVM, LVMI and left ventricular capacity-time curve parameters including Tmsv16-SD, Tmsv16-Dif, Tmsv16-SD/R-R(%), Tmsv16-Dif/R-R(%) significantly increased, while LVEF markedly decreased in the observation group (P<0.05). Conclusion RT-3DE is able to reflect left ventricular disfunction and asynchrony in type 2 diabetes with heart failure patients, and provide a reliable basis for clinical diagnosis.
