急性脑梗死患者S-100b蛋白的动态检测及其临床意义

目的动态监测急性脑梗死(ACI)患者血清S-100b蛋白的变化,探讨其与脑梗死体积、神经功能缺损程度及预后的关系。方法采用ELISA对50例颈内动脉系统ACI患者血清S-100b蛋白进行动态检测,并与30例对照组进行比较,同时进行神经功能缺损评分(NIHSS)及CT扫描测定脑梗死体积,随访2个月进行日常生活能力评分(BI)。结果ACI患者血清100b蛋白在入院第1d开始升高,第3d达到峰值,第7d下降,第14、21d与对照组无明显差异;第3d血清S-100b蛋白浓度高峰水平与脑梗死体积、神经功能缺损程度呈正相关,与2个月后BI呈负相关。结论急性脑梗死后血清S-100b蛋白可作为缺血性脑损伤的生化指标,并可评估预后,对脑梗死的临床治疗有指导意义。

Seriay assay and clinical value of S-100b protein in patients with acute cerebral infarction

Objective To analyze the changes of S-100b protein concentration in serum from patients with acute cerebral infarction and to find the correlation with neuralgic deficits,infarct volume and prognosis.Methods S-100b serum levels were serially determined in 50 patients with acute inschemic stroke in inner carotid artery system and in 30 control subjects.The neurological status was evaluated by the National Institutes of Health Stroke Scale(NIHSS)and Batrhel Index(BI).The cranial computed tomography(CCT)was performed to calculate infarct volume.Results The concentrations of S-100b were higher in patients with ACI,and peaked concentration of S-100b at 3th day after conset.They decreased gradually after the peak.There was no significant difference of serum levels of S-100b between control group and ACI group at 14,21th days.There was a positive correlation was also negatively related to BI at two months.Conclusions The level of S-100b in serum can be used as a marker of ischemic brain damage,may be helpful for therapeutic decision in acute ischemic stroke.
