肾移植术后发生泌尿系肿瘤与人类乳头瘤病毒感染的关系

目的 研究肾移植术后发生泌尿系统肿瘤与人类乳头瘤病毒（HPV）16、18的关系。方法 1974年1月至2005年1月间共进行同种异体肾移植3000例次，其中发生泌尿系统恶性肿瘤44例，包括5例肾细胞癌和39例泌尿系统移行上皮癌。采用聚合酶链反应（PCR）检测肿瘤组织中HPV16、18的感染情况，并与20例普通膀胱癌患者的肿瘤组织标本进行比较。结果 44例肾移植术后发生泌尿系统肿瘤的患者中有3例HPV阳性，其中1例膀胱癌为HPV16阳性（2．27％），1例双侧肾盂输尿管癌和1例膀胱癌为HPV18阳性（4．55％）；对照组中有1例为HPV18阳性（5％）；两组相比较，差异无统计学意义（P=1．000）。结论 肾移植术后发生泌尿系统肿瘤与HPV16、18的感染没有明显的相关性。

Relationship between the human papillomavirus 16, 18 and tumor of urinary system after renal transplantation

Objective To study the relationship between the human papillomavirus (HPV) type-16, 18 and tumor of urinary system after renal transplantation. Methods 3000 renal transplantations were done from Jan. 1974 to Jan. 2005. Among the 3000 patients, 44 were diagnosed as urological malignant tumor, including 5 cases of renal cell carcinoma, 39 cases of transitional cell carcinoma (TCC). By using the polymerase chain reaction method the HPV-16 and 18 infection was detected. Results The positive rates of HPV-16 and 18 were 2. 27% (1/44) and 4. 45% (2/44) respectively. Only 1 case in control group was proved positive for HPV-18 with the positive rate being 5% (1/20). Conclusion HPV may not play a major role in the occurrence and development of urinary tumor (transitional cell carcinoma) in renal transplant recipients.
