经口唇下蝶窦显微外科治疗垂体腺瘤

目的探讨垂体腺瘤的诊断和经口唇下蝶窦显微手术的方法及经验。方法经口唇下蝶窦显微外科切除垂体腺瘤30例(微腺瘤8例,大腺瘤18例,巨大腺瘤4例,侵袭性腺瘤16例),观察其治疗效果。结果肿瘤全切除26例(86.7%),次全切除4例,无死亡病例,术后并发症少。25例(83.3%)随访6个月至5年,22例(88%)痊愈,3例好转,1例复发。结论MR对选择手术入路有重要作用。经口唇下蝶窦显微手术是治疗垂体腺瘤的有效、安全的一种方法。

The treatment of pituitary adenomas with transsphenoidal microsurgery

Objective To explore the diagnosis of pituitary adenomas and sum up the experience of transsphenoidal ap-proach.Method Between1996and2001,clinical manifestation,the endocrine variation and radiological assessment (MRI and CT)for30patients of pituitary adenomas were reviewed.Their ages ranged from18to57years.Of the patients,8cases were microadenomas,18macroadenomas,4giant adenomas and16aggressive macroadenomas.All patients were performed via the sublabio-septo-sphenoidal approach.Result Of the patients,total adenomas excision was achieved in26patients and subtotal adenomas excision was achieved in4patients.Operative morbidity was low and operative mortality was0in this group.Long-term follow-up review ranging from6months to5years(mean3.5years)in25(83.3%)patients revealed cure of22(88%)patients,improvement of3and recurrence of1.Conculsion We considered that MR examination is important to di-agnos and select operative approach of pituitary adenomas.Transsphenoidal microsurgery is an important treatment for pituitary adenomas.
