EB病毒血清学检测和EB病毒DNA定量检测在诊断传染性单核细胞增多症中的应用价值

摘    要:目的探讨EBV血清学检测和EBV-DNA定量检测诊断传染性单核细胞增多症(IM)的应用价值。方法选取广州医学院第一附属医院2010年1月至2011年4月期间78例诊断为IM的病例作为研究对象,使用酶联免疫吸附(ELISA)方法检测患者血清中EBV四项抗体,同时使用荧光定量PCR(FQ-PCR)检测血浆中EBV-DNA负载量,对比两组的检测结果。结果抗EBV-VCA-IgM抗体阳性率93.6%,EBV-DNA阳性率为71.8%。结论 IM临床诊断中抗EBV-VCA-IgM阳性率比同期血浆中EBV-DNA阳性率高,故不推荐对IM患者进行常规EBV-DNA检测,但对病程长,症状反复,病情重的患儿应监测血浆中EBV-DNA,有助于评估治疗的效果及预后。

Abstract:Objective To investigate the EBV serological detection and quantitative EBV-DNA assay for the diagnosis of infectious mononucleosis ( IM ) application value. Methods The first hospital of Guangzhou Medical College in 2010January -2011 April78 cases were diagnosed as IM case as the research object, the use of enzyme-linked immunosorbent assay ( ELISA ) method for the detection of serum EBV four antibody, at the same time using fluorescence quantitative PCR (FQ-PCR) were detected in the plasma of EBV-DNA load, comparing the two groups of test results. Results Anti EBV-VCA-IgM antibody positive rate was 93.6%, the positive rate of EBV-DNA was 71.8%. Conclusion IM clinical diagnosis of EBV-VCA - IgM positive rate than the same period in the plasma of EBV-DNA positive rate is high, it is not recommended for patients with IM routine EBV-DNA testing, but for the long course of disease, symptoms of recurrent, severe illness in children should be monitored in plasma EBV-DNA, contribute to the evaluation of therapeutic effect and prognosis.
