狼疮肾炎患者外周血淋巴细胞早期凋亡率的分析

目的 探讨狼疮肾炎(lupus nephritis,LN)患者外周血淋巴细胞(peripheral blood lymphocytes,PBL)早期凋亡的改变与其自身抗体产生的相关性,并探讨淋巴细胞凋亡在LN发病机理中的作用。方法应用最新的Annexin V试剂盒及流式细胞仪(flow cytometry,FCM)对LN患者及年龄、性别相匹配的正常人PBL进行早期凋亡细胞的检测;并运用免疫印迹法检测LN患者组自身抗体谱及应用免疫浊度法检测补体C3含量。结果 ①LN患者PBL凋亡率较对照组明显增高(t=7.738,P<0.001)。②LN患者中,能测定出两种以上自身抗体阳性者凋亡率明显高于至多有一种自身抗体阳性者凋亡率(t=5.417,P<0.001);且活动期LN患者PBL凋亡率与补体C3含量呈高度负相关(r=-0.859,P<0.01)。结论LN患者PBL凋亡率明显高于正常人,这种凋亡的异常可能是LN发生发展的重要原因之一。

Study on the Rate of Early Apoptotic Lymphocytes in the Peripheral Blood of Patients with Lupus-nephritis

Objective To investigate whether the apoptosis of lymphocytes is related to autoantibody level in lupus-nephri-tis(LN)patients ,and inquire into the role of the apoptosis of lymphocytes in the pathogenesis of LN. Methods The levels of the early apoptotic cells of LN patients and control group,age and sex-matched healthy donors were assessed by a novel assay-Annexin V and propidiumiodide(PI)staining with flow cytometry (FCM); Anti-ENA autoantibodies of LN patients were detected with Western-blot ;and complement (C3)of the patients and the control group was tested quantitatively by turbidimetry. Results (1)The percentage of the early apoptotic lymphocytes in the peripheral blood of active LN patients was significantly higher than that of the normal healthy donors (t = 7. 738,P<0. 001). (2)Statistically significant differences were shown bet ween, one group with two and more kinds of autoantibodies determined .and another group which had at most one kind of autoantibody in LN patitents (t = 5. 41 7,P<0. 001). Besides,the rate of apoptosis negatively correlated with the complement in active LN patients(r= -0. 859,P<0. 01). Conclusions The early apoptotic lymphocytes increase significantly in the peripheral blood of human LN patients,the abnormal apoptosis may play an important role in the pathogenesis of LN.
