额下经眶小骨窗开颅显微手术治疗鞍区巨大肿瘤

摘    要:目的探讨额下入路经眶小骨窗开颅显微手术治疗鞍区巨大型肿瘤的方法与疗效。方法对35例位于鞍内、鞍上并向鞍区周围部位侵袭生长的巨大型肿瘤采用一侧额下入路经眶小骨窗开颅显微手术切除。结果肿瘤显微镜下全切除27例,次全切除8例,无一例死亡,无严重并发症,恢复良好。结论此入路应用显微外科技术切除鞍区巨大型肿瘤均可做到显微镜下全切除或次全切除,具有操作简便,显露良好,创伤小的微创优点,是治疗鞍区巨大型侵袭性肿瘤较理想的手术入路。

Abstract:Objective To investigate and method and effect of the microsugical technique for large sellar region tumors with unilateral transorbital small craniotomy through subfrontal lobe approach incision.Methods Thirty-five large sellar region tumors were all operated with unilateral transorbital small craniotomy through subfrontal lobe approach incision.The tumors were excised using microsurgical technique.Results The sellar region tumors were totally excised in 27,and sub-totally excised in 8.No patients led in peri-operative stage.No patients leads to serious complications.All patients recovered well after operation.Conclusion The large sellar region tumors may be totally or sub-totally excised with microsurgical techniques using unilateral transorbital small craniotomy through subfrontal lobe approach incision.Such approach provides a better tumor exposure and less brain damage.So it is a rational surgical approach to large sellar region tumors.
