应用自制侧孔EV导管针超声介入硬化治疗巧克力囊肿疗效的研究

目的 观察在超声引导下应用自制侧孔EV导管针硬化治疗巧克力囊肿患者的治疗效果。方法 选择临床确诊附件巧克力囊肿患者268例共计291个囊肿，在超声引导下应用PTC针无水乙醇硬化治疗巧克力囊肿70例75个囊肿（PTC针组），应用EV导管针无水乙醇硬化治疗76例85个囊肿（EV导管针组），应用自制侧孔EV导管针无水乙醇硬化治疗122例131个囊肿（自制侧孔EV导管针组），术后3、6、12个月超声检查记录三组治疗情况。以术后12个月内囊肿完全消失为治愈，并比较分析三组治愈率及囊肿治疗过程中重复硬化的次数。结果PTC针组治疗巧克力囊肿治愈率81．3％，EV导管针组治愈率94．1％，自制侧孔EV导管针组治愈率99．2％，三组间治愈率比较差异有统计学意义，且自制侧孔EV导管针组治疗附件巧克力囊肿过程中重复硬化次数明显减少。结论应用自制侧孔EV导管针无水乙醇硬化治疗巧克力囊肿具有较高的治愈率，操作轻便、安全可靠，值得推广应用。

Ultrasound-guided interventional sclerotherapy of ovarian chocolate cyst by self-made lateral pole EV needle

To evaluate the therapeutic effect of ultrasound - guided interventional sclerotherapy of ovarian chocaolate cysts by self - made lateral pole EV needle. Methods Two hundred and ninety - one ovarian chocolate cysts in 268 patients were confirmed in clinic. 75 cysts in 70 patients were treated by absolute alcohol sclerotherapy with PTC needle under ultrasound guidance, 85 cysts in 76 patients were treated by absolute alcohol sclerotherapy with EV needle, 131 cysts in 122 patients were treated by absolute alcohol sclerotherapy with self - made lateral pole EV needle. The treatment condition was recorded by ultrasonogrphy at 3 , 6 and 12 months after sclerotherapy. Criterion of cure was that the cyst disappeared completely 12 months after sclerotherapy. The curative rate and the number of repeated sclerotherapy of chocolate cysts were compared among three groups. Results The curative rate in PTC needle group, EV needle group and self - made lateral pole EV needle group was 81.3% , 94. 1% and 99.2%, respectively. There was significant difference among three groups, the number of repeated sclerotherapy was decreased obviously in self - made lateral pole EV needle group. Conclusion Self - made lateral pole EV needle used in absolute alcohol selerotherapy of ovarian chocolate cysts has high curative rate. It is a safe and convenient method with wide application prospect.
