焦虑、抑郁情绪对2型糖尿病疗效的影响

目的：探讨焦虑、抑郁情绪对2型糖尿病患者疗效的影响。方法：136例糖尿病患者治疗前根据抑郁自评量表（SDS）或焦虑自评量表（SAS）评分后分为焦虑、抑郁组64例（SDS或SAS≥50分）和对照组72例（SDS或SAS〈50分）;两组均给予胰岛素治疗1个月。于治疗前后进行空腹血糖（FBG）、餐后2h血糖（2hBG）、糖化血红蛋白（HbA1c）、超敏C-反应蛋白（hsC-RP）及SDS、SAS评分比较。结果：虽经胰岛素系统治疗,但伴有焦虑、抑郁情绪的糖尿病患者的FBG、2hBG及hsC-RP水平仍明显高于对照组（P〈0.05或P〈0.01）;胰岛素用量更多,血糖控制更难达标。结论：焦虑、抑郁情绪严重影响糖尿病的治疗与转归。

Comparative study on effect of anxiety or depression on the therapeutic efficacy in type 2 diabetes

Objective：To investigate the effect of anxiety and depression on the therapeutic efficacy in type 2 diabetes（T2DM）.Method：According to self rating depressive scale（SDS）and self rating anxiety scale（SAS）,136 T2DM patients were divided into study group（scores of SAS or SDS≥50,64cases）,and control group （scores of SAS or SDS50,72cases）before treatment.Both of two groups of patients were given insulin therapy for a month and the dosage of insulin was recorded.Fasting blood glucose（FBG）,2-hour post prandial blood glucose（2 hBG）,glucoted hemoglobin（HbA1c）,and high sensitivity C-reactive protein（hsC-RP）were compared before and after treatment,as well as scores of SAS and SDS,which were re-scored after treatment.Results：The existence of anxiety and depression in T2DM patients results in significant higher C-RP level and blood glucose level.These patients need more insulin to control their blood glucose.Conclusion：Anxiety,depression and other negative emotion seriously influence the treatment effect and prognosis of type 2 diabetes.
