彩色多普勒在无损伤性肝循环中的应用

本研究应用彩色多普勒超声对于３０例正常肝脏，３０例慢性肝病，２５例肝硬化，２２例原发性肝癌症的门静脉，脾静脉及肝静脉进行多种数据测量。结果显示慢肝时门脉血流量显著少于正常，慢肝，肝硬化，肝癌时门脉内径明显增宽，血流速度明显减低，肝静脉内径变窄，最大流速比平均流速增高显著，充血指数小于正常、笔者认为，应用彩色多普勒为无创伤性测量肝脏循环确定一种新的诊断标准，门脉血流量的测定，可作为慢肝和肝硬化的鉴别

Application of color Doppler in non-injurious determination of hepatic circulation

Various data of portal veins, splenic veins and hepatic veins were determined by Doppler sonograph in 30 normals, 30 patients with chronic hepatitis 25 pations with hepatocirrhosis and 22 patients with primary carcinoma of the liver.The resuts showed that the portal veins flow decreased markedly in the patients with chronic hepatitis, that the internal diameters of portal vein increased obviously in the patients with chronic hepatitis, hepatocirrhosis and liver cancer, that the blood flow speed of portal veins decreased remarkably, that the internal diameter of hepatic veins was narrowed. that the Vmax and Vmean increased obriously and that CI was lower than the normal. It is considered that a new kind of diagnostic criterion is offered by color Doppler for determining non-injurious hepatic circulation. that measuring portal vein flow is one of those important indices in distinguishing diagnosis of chronic hepatitis and hepatocirrhosis, that the Vmax, Vmean and CI of portal veins, splenic veins and hepatic veins reffected the condition of hepatic circulation, which played a important role in the judgment of diagnosis, therapy effect and prognosis in the patients.
