经腹腔镜逆行治疗复杂胆囊结石200例临床分析

目的：通过经腹腔镜逆行次全胆囊切除术和经腹腔镜逆行全胆囊切除术治疗复杂良性胆囊结石的对比，探讨经腹腔镜逆行治疗复杂胆囊结石的疗效。方法：我院从2011年1月至2012年6月共收治复杂性胆囊结石患者200例，随机分为观察组和对照组各100例，观察组患者给予经腹腔镜逆行次全胆囊切除术治疗；对照组患者给予经腹腔镜逆行全胆囊切除术治疗。分析比较两组患者的平均手术时间、术中出血量、中转开腹率及住院时间等情况。结果：两组患者术后中转开腹率、手术并发症及住院时间方面差异不大。观察组患者在手术时间、术中出血量、术中补液量等方面显著优于对照组，具有统计学意义（P〈0．05）。结论：结果显示，腹腔镜逆行胆囊次全切除术治疗复杂胆囊病变的手术时间较短，术中出血量少，疗效显著。

Clinical Analysis of Laparoscopic Retrograde on the Treatment of 200 Patients with Complicated Gallstone

Objective： Through the comparison of laparoscopic subtotal cholecystectomy retrograde and laparoscopic retrograde to cal effect of laparoscopic tal cholecystectomy to treat complex benign gallbladder stones, observing the cliniretrograde to treat complicated gallstone. Method： 200 patients with complicatedgallstone treated in our hospital from January 2011 to June 2012 were selected and divided into 2 groups, the observation group and the control group, each group for 100 cases. The observation group were given the treatment of laparoscopic subtotal cholecystectomy by treating retrograde, and the control group were treated with laparoscopic retrograde total cholecystectomy. Analyzing and comparing the the average operation time, the amount of bleeding in operation, conversion rate and hospitalization time of two groups. Result： There was no evident difference for conversion rate, operation complications and hospitalization time between two groups. The operation time, the amount of bleeding in operation and intraoperative fluid infusion of the ob- servation group were better significantly than that of those control group （P〈0. 051. Conclusion： The result indicates that the operation time of retrograde laparoscopic subtotal cholecystectomy to treat complicated gallstone is shorter, and the amount of bleeding in operation is less. The curative effect is evident.
