高龄初产妇的妊娠,分娩经过与妊娠结局探讨

探讨高龄初产妇妊娠经过，分娩过程及胎儿结局的影响。方法采用回顾性分析的方法。结果高龄初产妇在姓娠期合并症及并发症中，除妊高征的发生率明显高于对照组外，余两组间差异均显著意义；高龄初产妇的剖宫产率逐年增加；产程及异常分娩与对照组相比未见差异；高龄初产妇大胎儿双顶径方面显著低于对照组，而新生儿体重、Ａｐｇａｒ评分，围生儿病死率，羊水平段等方面差异均显著意义。结论高龄初产妇只要加强围生期健，定期进行产前

Investigation on pregnancy, delivery and pregnant outcome of elderly primipara

Objective To study the effect of maternal age on pregnancy,delivery and fetal prognosis. Methods A retrospective study was used, including 186 elderly primiparas and in the same time random sampling aged 20 -29 year primipawas as controls. Associations between maternal age and pregnancy outcome was assessed with t test. Results Incidence of pregnancy-induced hypertension among elderly primipara increased significantly to compare with control. However,statistical analysis of other pregnancy complications between tow groups was not different. The incidence of Cesarean section in elderly primiparas was increased year by year. There was not difference between two groups in dystocia of elderly primigaras. BDP of fetus in elderly primipara were lower evidently, Apgar score,perinatal death rate and so on was not different to compare with the control group. Conclusion So long as the elderly primiparas enhance perimatal health protection,carry on antenatal care periodicly,prevent actively and treat the PIH as soon as possible,they will undergo the pregnancy,delivery safely and gain a healthy infant.
