腹腔镜手术与开腹手术治疗子宫肌瘤113例临床分析

目的:探讨腹腔镜下子宫肌瘤切除术与剖腹子宫肌瘤切除术的临床效果。方法:选择同样手术指征的子宫肌瘤患者113例,分为腹腔镜手术组58例和剖腹手术治疗组55例,对两组手术时间、术中出血量、手术并发症、术后镇痛用药率、术后恢复情况、住院时间及疾病复发进行比较分析。结果:腹腔镜和剖腹手术组手术时间平均分别为65.48min、63.24min;术中出血量平均为38.32ml、118.70ml,术后并发症发生率分别为0和7.3%;术后镇痛用药率分别为3.6%、81.6%;住院时间平均为4.5d、12.6d,肛门排气时间平均为16.8h、42.16h,下床活动时间平均为6.18h、31.6h。两组手术时间无统计学意义(P>0.05),腹腔镜组出血量明显少于剖腹手术组(P<0.01)。结论:腹腔镜下子宫肌瘤切除术是一种安全、可靠、创伤性小的术式,值得推广应用。

Comparision of laparoscopic surgery and laparotomy in the treatment of uterine fibroids

Objective To investigate the clinical value of laparoscopic surgery and laparotomy in the treatment of uterine fibroids.Method 113 cases with uterine fibroids were randomly divided into the laparoscope group(58 cases) and the laparotomy group(55 cases).The intraoperative time and blood loss,postoperative complications,application rate of anodyne and recovery in the two groups were compared.Results The average operative time was 65.48 min in the laparoscope group and 63.24 min in the laparotomy group(P>0.05),and the mean operative blood loss was 38.32 ml and 118.7 ml(P<0.01).The occurrence rate of postoperative complications were 0 and 7.3%.The application rate of anodyne were 3.6% and 81.6%.The average time in hospital were 4.5d and 12.6d.The average evacuating time were 16.8 h and 42.16 h.The average exercise time were 6.18 h and 31.6 h.Comparing to laparotomy,laparoscopic surgery had the advantage of minimally invasive,fewer blood loss,short time in hospital,recovery rapidly and lower rate of complications.Conclusion Laparoscopic myomectomy is safe,reliable and little microtraumatic therapy in the treatmet of uterine fibroids.
