芜湖市2012年死亡病例报告质量分析

目的对芜湖市死亡病例网络报告系统报告质量进行评估。方法收集2012年芜湖市网络直报死亡病例,使用报告及时率、审核及时率、常见编码错误分布等指标对芜湖市2012年死亡病例报告情况进行评价。结果 2012年芜湖市共报告死亡病例8263例,院内死亡病例所占比例为43.30%,院外死亡病例56.70%;报告及时率为84.04%,院内死亡病例报告及时性高于院外死亡病例(χ2=374.27,P<0.01);审核及时率为99.66%;有明显编码错误的比例为24.01%,其中院外死亡病例编码错误率为33.66%,院内死亡病例为11.38%,差异有统计学意义(χ2=551.90,P<0.01)。结论死因登记报告工作目前存在比较普遍的问题是死因编码质量普遍偏低,根本死因确定和编码培训工作亟待加强。

Analysis on the quality of death-case-reporting-system of 2012 in Wuhu city

Objective To assess the quality of network-based death case reporting system in Wuhu city. Methods Data of death cases reported directly in the network-based system were collected, analyzed, and the quality of death-case-reporting-system was evaluated in terms of the promptness of reporting and auditing and common coding error. Results A total of 8263 death cases were reported in 2012 in Wuhu city, in which 43.30% of death cases were from medical institutions, whil the remaining cases were from outside. The overall promptness rate of reporting was 84.04 %, whiche was much higher among death cases reported from medical institutions compared to those from outside （Χ^2= 374.27, P〈0.01） . The timely rate of auditing was 99.66 %. 24.01 % death cases contained obvious coding errors which was three times higher among death cases reported outside medical institutions （33.66 %） compared to cases reported inside （11.38 % ）, and the difference was statistically significant （Χ^2 =551.90, P〈0.01）. Conclusions The quality of coding on death causes in medical institutes is poor, suggesting that the training on ascertainment and coding of underlying death causes should be improved.
