我院Ⅰ类切口手术患者围术期抗菌药物使用情况分析

目的 调查笔者所在医院Ⅰ类切口手术围术期预防性应用抗菌药物的情况. 方法 对笔者所在医院2011年9-12月骨科、普外科择期Ⅰ类切口手术的出院病历203份进行回顾性分析. 结果 围术期预防性抗菌药物使用率为100%;超使用级用药占68.93%,超类别用药占34.85%;单一用药占80.30%,二联用药占11.33%,手术前后更换药物占8.37%;术前2 h内给药占25.62%,术前未给药占13.30%,术前>2 h给药占35.96%,术后持续给药时间≤24 h占25.62%,给药时间＞24 h占74.38%. 结论 Ⅰ类切口手术围术期预防性应用抗菌药物存在药物使用率高、药物选择和用药时机不规范等问题,需制定相应措施规范围术期抗菌药物的合理应用.

Analysis of Antibacterials Use in Patients with Class I Incision during Perioperative Time in Our Hospital

Objective To investigate the prophylactic use of antibiotics in class I incision during perioperative time in our hospital. Methods Clinical data of 203 patients who would undergo class I incision operation in General Surgery and Osteology Department from September to December 2011 in a hospital were retrospectively analyzed. Results The perioperative utilization rate of antibiotics was 100% , there were 68.93% of Ultra-use class use, 34.85% of Uhra- classes use, 80.30% of single usage, 11.33% of bigeminy usage, 8.37% of replacement use before or after surgery, 25.62% of medication within 2 hours before surgery, 13.30% of without medication before surgery, 35.96% of medication earlier than 2 hours before surgery, after surgery 25.62% of continue medication no more than 24 hours, and 74.38% of more than 24 hours. Conclusion There are some problems of use of antibiotics in class I incision during perioperative time such as high rate of antibiotics usage, non-standard selection and non-standard time of use. To ensure the antibiotics can be rationality used, the countermeasure in peroperative period should be enacted.
