摘    要:目的 探讨子宫内膜异位症导致不孕的免疫机理。方法 选择临床轻度子宫内膜异位症不孕患者为研究对象,以多囊卵巢综合征及不明原因不孕的正常盆腔为对照。采取ELISA检测腹腔液IL—6水平,以流式细胞分选技术检测外周血T淋巴细胞亚群的变化。结果 与对照组相比,轻度子宫内膜异位症不孕患者腹腔液IL—6水平显著增高,外周血CD4^ T细胞数目增加,而CD8^ T细胞变化不明显。结论 轻度子宫内膜异位症不孕患者腹腔免疫细胞处于异常活化状态、体内存在免疫调节异常,在局部表现特别明显。提示IL—6可能通过直接细胞毒或协同作用干扰生殖活动多个环节,从而在子宫内膜异位症导致不孕中发挥重要作用。

Abstract:Objective To explore the immune mechanism of endometriosis induced infertility.Methods The infertile women with mild endometriosis were chosen and those with polycystic ovarian syndrome and unidentified causes as controls. The levels of IL 6 in peritoneal fluid were determined with ELISA, and the changes of T lymphocyte subpopulation in peripheral blood was also determined with flow cytometry technique. Results IL 6 level was obviously increased in the infertile women with mild endometriosis compared with the controls. CD4 + T cell number in peripheral blood was increased while CD8 + T cell number had no evident changes. Conclusion The peritoneal immune cells were in the state of hyper activated activation and there was dysimmunity in the infertile women with mild endometriosis. It suggests that IL 6 may play a role in endometriosis induced infertility.
