维持性血液透析患者左旋肉碱水平与血脂关系的研究

目的　观察维持性血液透析 (MHD)患者血清游离肉碱 (SFC)水平与血脂的关系。方法　采用随机对照的研究方法选择MHD患者 6 0例 ,分为治疗组 30例、对照组 30例 ,每次透析结束后 ,治疗组静脉注射左旋肉碱 1.0g ,对照组静脉注射生理盐水 5ml,疗程共 12周。应用紫外酶法检测其透析前后的SFC ,常规方法检测总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇。结果　治疗组SFC水平显著升高 (P <0 .0 1)、总胆固醇、甘油三酯较治疗前显著降低 (P <0 .0 5 ) ,高密度脂蛋白胆固醇有显著升高 (P <0 .0 5 ) ,低密度脂蛋白胆固醇无显著性变化 ,对照组血脂四项均无显著性变化。结论　MHD患者普遍存在左旋肉碱缺乏及血脂代谢紊乱 ,补充左旋肉碱可明显提高脂肪利用 ,改善脂代谢紊乱的情况

Investigation of serum free carnitine level in maintenance hemodialysis patients and its relation to their serum lipid level

Objective To observe serum free carnitine(SFC) level in maintenance hemodialysis patients and its relation to their serum lipid level. Meathods Sixty maintenance hemodialysis patients were randomly divided into 2 groups, therapy group:L-carnitine 1.0g intravenously infuse after every dialysissession;control group were given saline 5ml. We observed for 12 weeks. The concentration of SFC was tested with ex-violet enzyme technique, and the other index were tested with routine method. Results In therapy group,the concentration of SFC was significantly increased (P<0.01), total cholesterol and triglyceride was decreased (P<0.05),and high density lipoprotein (HDL) in value was higher than in control group (P<0.05).Low density lipoprotein (LDL) did not change significantly, and there is also no significant change of serum lipid level in control group. Conclusion In maintenance hemodialysis patients, L-carnitine absence and metabolism disorder of serum lipid are widespread and its complementation can singnificantly increase the use of fat and improve the disorder condition
