不同剂量布托啡诺复合芬太尼用于全麻术后静脉镇痛的临床观察

目的:观察不同剂量布托啡诺复合芬太尼的镇痛效果以及不良反应,寻求一种安全,有效的术后静脉镇痛配方。方法:90例全麻病人,随机分为3组,每组30例。Ⅰ组,布托啡诺8mg+芬太尼0.4mg;Ⅱ组,布托啡诺6mg+芬太尼0.6mg;Ⅲ组,布托啡诺4mg+芬太尼0.8mg。3组均以生理盐水稀释至100ml。术后开始PCIA治疗,并观察各组的镇痛效果和不良反应情况。结果:3组镇痛效果差异无显著性意义(P>0.05);恶心呕吐,Ⅲ组多于Ⅱ组(P<0.05);嗜睡,Ⅲ组与Ⅱ组或Ⅰ组间差异有显著性意义(P<0.05);总的不良反应发生率Ⅰ组和Ⅲ组均多于Ⅱ组(P<0.05)。结论:Ⅱ组配方镇痛效果满意,不良反应发生率也相对较少。

Clinical Observation of Different Dosages of Butorphanol plus Fentanyl in PCIA after General Anaesthesia Surgery

Objective： To observe the analgesia effect and adverse effect of different dosages of Butorphanol plus Fentanyl. Methods： 90 cases receiving general anaesthesia surgery were randomly divided into 3 groups, 30 cases in each. Group Ⅰ , 8mg of Butorphanol ＋ 0.4mg of Fentanyl; Group Ⅱ, 6mg of Butorphanol ＋ 0. 6mg of Fentanyl; Group Ⅲ, 4mg of Butorphanol ＋ 0.8mg of Fentanyl, all the drugs were diluted in Sodium Chloride to 100ml. PCIA was given to all the patients after they awoke from general anaesthesia, and observe the analgesia .effect and adverse effect. Result： There was no significant difference among the three groups in analgesia effect（P〉 0.05）. The nausea/vomiting in Group Ⅲ was higher than that in Group Ⅱ （P〈0.05）. The lethargy between Group Ⅲ and Group Ⅱ/ Group Ⅰ has significant difference（P〈0.05）. Conclusion. The formula of Group Ⅱ has satisfactory analgesia effect and the lower adverse effect incidence, so it is of practical clinical significance.
