硬膜外阻滞对瑞芬太尼丙泊酚静脉全麻苏醒期心血管反应的影响

目的评价硬膜外阻滞对瑞芬太尼、丙泊酚静脉全麻苏醒期心血管反应的影响。方法 60例择期行腹部手术患者,随机分为瑞芬太尼、丙泊酚静脉全麻组（A组）和瑞芬太尼、丙泊酚静脉全麻复合硬膜外阻滞组（B组）,每组30例。观察全麻苏醒期血压、心率变化,术毕恢复时间及术后疼痛视觉模拟评分（VAS）等。结果 A组苏醒期MAP、HR较术中明显升高,B组苏醒期MAP、HR较平稳,术毕恢复时间及VAS评分均优于A组（P〈0.05）。结论硬膜外阻滞可抑制全麻苏醒拔管期的心血管反应,苏醒平稳、安全可行。

The Effect of Epidural Block on Cardiovascular Responses During Recovery Period of Remifentanil-Propofol General Anesthesia

Objective To evaluate the effect of epidural block on cardiovascular responses during the recovery period of remifentanil-propofol general anesthesia.Methods Sixty ASA status I～II patients undergoing abdominal surgery were randomly divided into one of two groups to receive general anesthesia （group A） or general anesthesia combined with epidural block （group B）.The hemodynamic variables during operation,the time from discontinuation of propofol to eyes-openning, to tracheal extubation, and to leaving the operating-room were observed,and postoperative pain was recorded with VAS.Results During recovery period,the MAP and HR increased significantly in group A,but maintained stable in group B. The time from discontinuation of propofol to eyes-openning,to tracheal extubation, and to leaving the operating-room were significantly shorter in group B than that in group A.The VAS in recovery period was higher in group A than that of group B.Conclusion The extradural block can suppress the cardiovascular responses to tracheal extubation,and maintain stable in hemodynamic variables.
