超声造影对膀胱癌的诊断价值

目的研究超声造影剂（SonoVue）在膀胱癌分期诊断中的应用价值。方法对17例20个经临床确诊并预行经尿道切除或膀胱全切的膀胱肿瘤患者进行灰阶超声造影检查，判断其分期并与病理结果对照。结果T1期12个于癌灶所在处见膀胱壁结构，强化肿瘤与膀胱壁间有线样低密区，判断为未侵犯肌层；T2期4个癌灶，局部外缘光整自然，为浅肌层受累，3个癌灶局部外缘不光整或僵硬，判断为深肌层受累T3期；1个局部外缘毛糙，脂肪问隙与癌肿有相近程度强化，判断为穿透外膜T4期。超声造影提示膀胱癌的分期与病理分期的总符合率达95．0％，对T1、T3、T4期膀胱癌的判断准确率为100％。结论灰阶超声造影做为无创手段为膀胱癌的分期诊断提供了新的依据。

The value of contrast-enhanced ultrasound in diagnosing bladder cancer

Objective To study the value of ultrasound contrast agent SonoVue in diagnosing bladder cancer. Methods A total of 17 patients with 20 tumors who selected for transurethral resection or cystectomy based on the clinical dianosis willing to have contrast -enhanced ultrasound （CEU） were scanned by CEU. The invasive degree of bladder wall of enhanced cancer were evaluated and analyzed by which juge the stage of bladder cancer, and compared with pathology. Results CEU showed 12 cancers of T1 were seen the local bladder wall structures in which there were low density line between enhanced tumors and bladder wall that were juged noninvasive muscle layer; 4 cancers of stage T2 whose local basic line were smooth and nature that were juged invasive superficial muscle. The local basic line of 3 cancers of stage T3 were irregular that were juged invasive deep muscle ; And 1 cancer whose local line was coarse and there was the same enhanced between the fat interval and cancer that was judged penetrate the outer layer. The accuracy of contrast - enhanced ultrasound in the deciding the stage of bladder cancer was 95.0%. Conclusions CEU as a noninvasive method can provide a new basis for diagnosis the stage of bladder cancer.
