ELISA、胶体硒和免疫试鸭验检测HIV抗体结果分析

目的 复检和确认初筛HIV抗体阳性血清，并对比分析试验结果。方法 对初筛阳性血清，用胶体硒和ELISA试验复检，任一复检试验阳性的血清再用westernblot（WB）试验确认。结果 224份初筛阳性血清经复检84份阳性，WB确认61份HIV—1抗体阳性，不确定10份，阴性13份。以WB结果为标准，ELISA、胶体硒法的阳性符合率分别为84．72％和82．43％。ELISA的敏感性为100％，特异性为96．84％，胶体硒法的敏感性为100％，特异性为95．03％。阳性、不确定和阴性组血清EI，ISA平均S／CO分别为20．550，2．244和1．434。ELISA和胶体硒法复检同时阳性，在阳性组、不确定组、阴性组分别为100％（61／61），10％（1／10）和0（0／13）。结论 胶体硒和ELISA复检可排除大部分初筛假阳性，但两种复检试验均存在一定的假阳性。3种试验的不符情况仅出现在HIV抗体阴性和不确定组，确定HIV感染依赖于WB确认试验。

Comparison on ELISA , Electroselenium method and Western blot for detection of anti-HIV antibody

Objective To re-screen and confirm anti-HIV antibody positive serum in primary screening-test,and to compare and analyze results of re-screening tests and Western blot(WB).Methods Samples positive for anti-HIV antibody in primary screening test were re-screened with both Determine HIV1 2 kit(Electroselenium method) and Vironostika HIV uni-form II Ag/Ab(ELISA) kit.Positive samples in either re-screening test were further confirmed with WB.Results Out of 224 samples positive in primary screening-test,84 were positive in re-screening test.61 were positive,10 suspected and 13 negative in WB.In contrast to WB,positive coincidence rates of ELISA and Electroselenium method were 84.72%(61/72) and 82.43%(61/74) respectively.The sensitivity and specificity for ELISA was 100% and 96.84% respectively,and for Electroselenium method 100% and 95.03% respectively.In ELISA re-screening test,the mean S/CO value for WB positive,suspected and negative samples were 20.550,2.244 and 1.434 respectively.Out of 61 positive samples,10 suspected samples and 13 negative samples in WB,61,1 and 0 samples were positive in both re-screening tests i.e.100%,10% and 0 respectively.Conclusion Anti-HIV antibody re-screening tests could exclude most false positive samples in primary screening test.However,there is still some false positive rate in either re-screening test.Discordance occurs only in suspected or negative samples.Confirmation of HIV infection relies on WB.
