血清miR-27a水平与多囊卵巢综合征及其临床特征的相关性

摘    要:目的:分析多囊卵巢综合征(PCOS)患者血清miR-27a水平及其与临床特征的相关性。方法:收集2018年10月—2019年10月在郑州人民医院就诊的PCOS患者96例(PCOS组)、健康女性60名(对照组)为研究对象,qRT-PCR检测两组血清miR-27a水平,超声检测两组受试者卵泡数和卵巢体积,采血检测卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇、孕酮、睾酮雌激素5项,同时检测空腹血糖及空腹胰岛素(FINS),计算稳态模型评估-胰岛素抵抗指数(HOMA-IR)。比较两组各指标差异,Pearson相关性分析PCOS组miR-27a与临床指标的相关性,受试者工作特征(ROC)曲线及多因素Logistic回归分析miR-27a与PCOS的相关性。结果:PCOS组血清-27a水平显著高于对照组(t=6.658,P<0.05);PCOS组miR-27a水平与卵泡数、LH、睾酮、空腹血糖、HOMA-IR呈正相关(r=0.210、0.242、0.323、0.253、0.216,均P<0.05),与雌二醇呈负相关(r=-0.347,P<0.05);miR-27a诊断PCOS的曲线下面积为0.786(95%CI:0.716~0.848),miR-27a水平是PCOS的独立相关因素(OR=6.804,95%CI:2.342~19.763,P=0.000)。结论:miR-27a在PCOS患者血清中高表达,且与PCOS的多个临床特征密切相关,可能参与PCOS的发病及进展。

Abstract:Objective: To analyze the level of serum miR-27a in patients with polycystic ovarian syndrome(PCOS) and its correlation with clinical characteristics. Methods: 96 cases of PCOS patients(PCOS group) and 60 cases of healthy women(control group) who visited People′s Hospital of Zhengzhou from Oct,2018 to Oct,2019 were collected as subjects.The levels of miR-27a in serum were detected by qRT-PCR. The follicular number(FN) and the volume of ovaries(OV) in the two groups were detected by ultrasound in the two groups. Blood samples were collected to detect the follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol,progesterone,testosterone and estrogen. Fasting blood glucose(FPG) and fasting insulin (FINS) were measured and homeostasis model assessment of insulin resistance(HOMA-IR)were calculated. The differences of all indexes between the two groups were compared. The correlation between miR-27a and clinical data in the PCOS group were analyzed by Pearson correlation analysis.The correlation between miR-27a and PCOS were analyzed by ROC curve and multivariate Logistic regression analysis. Results: The level of serum miR-27a in the PCOS group was significantly higher than that in the control group(t=6.658,P<0.05). The level of miR-27a in the PCOS group was positively correlated with the follicular number,LH,testosterone,FPG,HOMA-IR(r=0.210,0.242,0.323,0.253,0.216,all P<0.05) and negatively correlated with estrogen(r=-0.347,P<0.05).The area under the curve of miR-27a in diagnosing PCOS was 0.786 (95%CI: 0.716-0.848),and miR-27a level was an independent correlation factor of PCOS(OR=6.804,95% CI:2.342-19.763,P=0.000). Conclusion: miR-27a is highly expressed in the serum of patients with PCOS and is closely related to multiple clinical features of PCOS,which might be involved in the pathogenesis and progression of PCOS.
