甲状腺癌侧颈淋巴结清扫术后并发乳糜漏的比较研究

目的:探讨甲状腺癌侧颈淋巴结清扫术后并发乳糜漏的临床特点、原因。方法:收集2014年1月—2019年3月于天津医科大学总医院普通外科行甲状腺癌侧颈淋巴结清扫手术患者的临床资料，根据患者术后是否出现乳糜漏分为乳糜漏组（7例）和对照组（140）例，将两组患者的临床资料进行回顾性对照分析。结果:乳糜漏组和对照组体重、体重指数、颈清侧别、淋巴结清扫个数、淋巴结转移个数、是否清扫Ⅴ区淋巴结、是否清扫Ⅵ区淋巴结、Ⅳ区淋巴结有无转移、Ⅴ区淋巴结有无转移、Ⅵ区淋巴结有无转移，差异均无明显统计学意义（均P＞0.05）。结论:患者的体重指数、颈清侧别、淋巴结清扫和转移个数、淋巴结清扫和转移区域对甲状腺癌侧颈淋巴结清扫术后是否发生乳糜漏没有影响。

Comparative study of chyle leakage after lateral neck lymph node dissection for thyroid cancer

Objective: To investigate the clinical characteristics and causes of chyle leakage after lateral neck lymph node dissection for thyroid cancer. Methods: The clinical data of patients undergoing lateral neck lymph node dissection for thyroid cancer in general surgery department of General Hospital of Tianjin Medical University from January 2014 to March 2019 were collected for retrospective analysis. According to whether there was chyle leakage post operation or not，the patients were divided into chyle leakage group （7 case） and control group（140 case）. Results:There were no significant differences between the two groups in clinical characteristics including weight，body mass index（BMI），the side of lateral neck lymph node dissection，the number of dissected lymph nodes，the number of metastasis lymph nodes，whether levelⅤlymph node dissection or not，whether level Ⅵ lymph node dissection or not，whether level Ⅳ lymph node metastasis or not，whether level Ⅴ lymph node metastasis or not，and whether level Ⅵ lymph node metastasis or not（all P＞0.05）. Conclusion: The patients′ BMI，the side of lateral neck lymph node dissection，the number of lymph nodes that dissected or metastasized，the lymph node dissection and metastasis area have no effect on whether chyle leakage occurs after lateral neck lymph node dissection for thyroid cancer.
