河南省郸城县县域医共体建设机制及成效分析

郸城县通过重塑医疗卫生服务体系，创新医疗卫生管理体制机制，完善医疗保障支持政策，形成党委统揽、政府主导的医疗卫生服务共同体，下设以县人民医院、中医院、中心医院、妇幼保健院为牵头医院的4个医疗健康服务集团，突出各集团的专科优势，集团间形成专科错位发展、良性竞争模式，进而引导患者合理有序就医。通过医共体建设，促进县级优质资源下沉基层，有效引导群众到基层医疗机构就诊，不但控制了医疗卫生费用的不合理增长，减轻了群众的就医负担，也在一定程度上缓解了“看病难”和“看病贵”难题。通过医疗体制改革，郸城县探索出一条“以人民健康为中心”的医改之路，持续提高县域医疗保障水平及全民健康素养水平，提升了县域医疗服务能力，实现了由“县级强”到“县域强”，形成了“县强、乡活、村稳”的新格局。

Construction Mechanism and Effect Analysis of County Medical Community in Dancheng County,Henan Province

Dancheng County reshaped the medical and health service system, innovated the medical and health management system and mechanism, improved the medical security support policies, and formed a medical and health service community dominated by the Party Committee and the government. It included four medical and health service groups which took the county people's hospital, hospital of traditional Chinese medicine, central hospital, maternal and child health care hospital as the lead hospitals, highlighting the specialty advantages of each group, the formation of dislocation of specialty development between groups, benign competition mode, and then guiding patients to a reasonable and orderly medical treatment. Through the construction of the medical community, the county level high-quality resources were promoted to sink into the grassroots, and the masses were effectively guided to the grassroots medical institutions, which not only controlled the unreasonable growth of medical and health costs, reduced the burden of medical treatment for the masses, but also alleviated the "difficult" and "expensive" problems of the masses to a certain extent. Through the reform of medical system, Dancheng County explored a reform road of "people's health as the center", continued to improve the level of the county medical insurance and national health quality, promoted the county health service ability, realized the "county-level" to the new situation of "county-territory", set up a new pattern of "strong county, alive township and stable village".
