BMI对超声诊断异位妊娠的影响

摘    要:目的探讨BMI对超声诊断异位妊娠的影响。方法回顾性分析2015年11月~2019年11月院内收治的异位妊娠患者,经筛选标准共纳入62例研究对象,均行超声诊断,根据BMI大小将研究对象分为两组:正常BMI组(18.5 kg/m2 < BMI < 25 kg/m2,n=43)和高BMI组(BMI≥25 kg/m2,n=19),比较不同分组患者的基线资料;比较不同BMI分组间行超声诊断异位妊娠影像学表现。结果两组患者基线资料的差异无统计学意义(P > 0.05);与正常BMI组相比,高BMI组超声诊断的阳性率更低,差异有统计学意义(P < 0.05);与正常BMI组相比,高BMI组超声诊断假孕囊、盆腔积液、宫外混合性团块阳性率更低,差异有统计学意义(P < 0.05),而胚芽、胎心诊断阳性率在两组间的差异无统计学意义(P > 0.05)。结论应用超声诊断异位妊娠患者时,应考虑BMI对超声诊断阳性率的影响,建议行其他方式诊断肥胖患者的异位妊娠。

Influence of body mass index on ultrasonic diagnosis of ectopic pregnancy

Abstract:ObjectiveTo investigate the influence of BMI on ultrasound diagnosis of ectopic pregnancy.MethodsA total of 62 patients with ectopic pregnancy who were admitted to our hospital and diagnosed by ultrasound from November 2015 to November 2019 were included and retrospectively analyzed according to the screening criteria. According to the value of BMI, the patients were divided into the normal BMI group (18.5 kg/m2 < BMI < 25 kg/m2) and the high BMI group (BMI≥25 kg/m2). The clinical baseline data of all patients were analyzed, and the baseline data of patients in different groups were compared. The imaging findings of ectopic pregnancy were compared between different BMI groups.ResultsThe difference in baseline data between the two groups was not significant (P > 0.05). Compared with the normal BMI group, the positive rate of ultrasound diagnosis in the high BMI group was significantly lower(P < 0.05). Compared with the normal BMI group, the positive rate of ultrasound diagnosis of phantom pregnancy bursa, pelvic effusion and mixed mass was lower in the high BMI group (P < 0.05). However, the difference in the diagnosis rate of germ and fetal heart between the two groups was not significant(P > 0.05).ConclusionWhen using ultrasound to diagnose patients with ectopic pregnancy, the influence of BMI on the positive rate of ultrasound diagnosis should be considered, and other methods are recommended to diagnose ectopic pregnancy in obese patients.
