MRI对椎管内硬膜外海绵状血管瘤的诊断价值

目的:探讨发生在椎管内硬膜外的海绵状血管瘤MRI影像学特征,提高对该类病变的认识。方法:搜集2001年~2007年经手术病理证实的5例发生在椎管内硬膜外的海绵状血管瘤的患者资料,进行回顾性分析。结果:发生在椎管内硬膜外的海绵状血管瘤5例,T1WI呈均匀或不均匀低信号,T2WI呈均匀高信号,增强扫描瘤体明显均匀强化,2例病变轴位显示病灶跨过左侧椎间孔向椎管外"钳状"生长。其中1例相应椎体合并小血管瘤。结论:椎管内硬膜外海绵状血管瘤为少见肿瘤,MRI对椎管内海绵状血管瘤的定位及定性有着无可替代的优势,是目前最为可靠的术前检查手段,能充分显示病变与周围组织的关系,但最后确立诊断,仍须结合临床及完整的病理资料分析。

Diagnostic value of MRI in spinal epidural cavernous hemangioma

Objective:To discuss MRI characteristics of spinal epidural cavernous hemangioma so as to improve the cognition of this tumor.Methods:5 patients with spinal epidural cavernous hemangioma as confirmed by operation and pathologiccal examination from 2001 to 2007 were retrospectively analysed.Results:Of the 5 cases,the masses showed uniformity or nonuniform low signal intensity on T1WI whereas hyperintense on T2WI.On the post-contrast image,the masses were uniformly enhanced in casse.Axial view in 2 cases displayed the tumor gawth like a clamp out of the vertebral canal through left side of the foramen intervertebrale.One case accompanied by one hemangioma in the vertebral body.Conclusion:Spinal epidural cavernous hemangioma are very rare tumor in the epidural space.MRI is the best orientation and can determine the nature for the diagnosis of this tumor in vertebral canal.It is the most dependable examination before operation at present,which can display the relationship of tumor and architectonic around.But the correct diagnosis depends on clinical and pathological results.
