阴式子宫肌瘤剔除术与腹腔镜下子宫肌瘤剔除术的临床疗效对比观察

目的对比观察阴式子宫肌瘤剔除术与腹腔镜下子宫肌瘤剔除术的临床疗效。方法选取2012年6月至2014年6月我院接诊的110例子宫肌瘤患者作为研究对象,按照随机数字表法将其分为观察组和对照组,每组各55例。对观察组予以阴式子宫肌瘤剔除术治疗,对照组予以腹腔镜下子宫肌瘤剔除术治疗,观察并比较两组临床疗效,术中平均出血量,手术时间、术后肛门排气时间、平均住院时间、平均住院费用及并发症发生情况。结果两组在临床疗效及并发症发生率方面无显著性差异(P>0.05);观察组患者平均手术时间、肛门排气时间均短于对照组,平均住院费用少于对照组,组间比较差异显著(P均<0.05);两组在术中平均出血量、平均住院时间方面无显著性差异(P>0.05)。结论与腹腔镜手术相比,经阴道手术操作快速精确,且较为经济,但阴道空间狭窄,易损伤邻近脏器,因而临床应根据患者情况合理选择手术方法,以达到理想疗效。

Comparsion and Observation of Clinical Effect of Vaginal Myomectomy and Laparoscopic Myomectomy

Objective To compare and observe the clinical efficacy of vaginal myomectomy and laparoscopic myomectomy. Methods 110 cases of patients with uterine fibroids treated in our hospital from June 2013 to June 2014 were selected and randomly divided into observation group and control group, with 55 cases in each group. The observation group received vaginal myomectomy, the control group received laparoscopic myomectomy. The clinical efficacy, the mean intraoperative blood loss, the operation time, the anal exhausting time after operation, the average hospitalization time, the average hospitalization cost and the complications were compared between two groups. Results The clinical efficacy and the incidence of complications of two groups had no significant difference （P〉0.05）;the operation time, anal exhausting time after operation of the observation group were shorter than those of the control group, the average cost of hospitalization of the observation group was less than that of control group, with statistically significant difference （all P〈0.05）;the mean intraoperative blood loss and the average hospitalization time of two groups had no significant difference （P〉0.05）. Conclusions Compared with laparoscopic surgery, vaginal operation enjoys operational speediness, accuracy and economy, but the vaginal space is narrow, adjacent organs is injured easily, so we should choose surgical methods based on patients＆#39; condition in clinic, thus getting curative effect.
