降钙素原早期诊断急性脑梗死合并医院获得性肺炎的临床应用

目的探讨降钙素原(PCT)早期诊断急性脑梗死合并医院获得性肺炎(HAP)的临床意义。方法选取我院神经内科因急性脑梗死入院,住院期间合并出现HAP的49例患者为实验组,未合并HAP的48例患者为对照组,所有患者入院当时及入院后1周内每天进行肺部感染筛查,并抽血检测PCT、CRP、WBC水平,比较诊断HAP前后两组的PCT水平。结果入院后第1天起实验组PCT明显高于对照组(P<0.01);实验组第2天的PCT值与第1天比较,无明显统计学差异(P>0.05),第3、4、5、6、7天的PCT值比前两天明显升高(P<0.01)。ROC曲线确定PCT诊断HAP的临界点为0.35μg/L,具有较高的灵敏度和特异度。结论降钙素原可以早期发现急性脑梗死合并医院获得性肺炎,为及时采取有效的预防和治疗措施创造有利条件。

Application of Procalcitonin in Early Diagnosis of Acute Cerebral Infarction Complicated with Hospital Acquired Pneumonia

Objective To explore the clinical significance of procalcitonin（PCT） in the early diagnosis of acute cerebral infarction（ACI） complicated with hospital acquired pneumonia（HAP）. Methods 49 patients with ACI treated in our hospital complicated with HAP were selected as experimental group, other 48 ACI patients without HAP were selected as control group. Pulmonary infection was screened and PCT, CRP, WBC levels were detected. PCT levels of the two groups before and after diagnosing HAP were compared. Results After admission, the PCT level of the experimental group was significantly higher than that of the control group from 1stday. The PCT level of the experimental group on 3rd, 4th, 5th, 6th, 7thday was significantly higher than the level on 1st, 2ndday. The diagnostic kit for HAP was determined at 0.35 μg/L by ROC curve. It had high sensitivity and specificity. Conclusions PCT can serve to find HAP early in patients with acute cerebral infarction to help taking preventive and treatment measures effectively.
