MSCT诊断先天性肺静脉异位引流临床价值

目的评价多排螺旋CT(MSCT)血管造影在先天性肺静脉异位引流中的临床价值。方法收集2005年至2009年94例MSCT诊断的肺静脉异位引流患者,其中78例经手术证实,(男性40例,女性38例)进行回顾性分析,并将CT诊断结果与术后诊断对照。结果 78例行手术治疗的肺静脉异位引流病例中,完全性肺静脉异位引流者共29例,其中心上型7例、心脏型14例、心下型3例、混合型5例;部分性肺静脉异位引流者共49例。与术后诊断相比,诊断正确符合率100%。结论 MSCT诊断肺静脉异位引流具有极高的诊断准确率,是一种很好的无创检查方法,一次检查不仅解决心内结构连接异常外,还可同时了解合并畸形或其它并发症的诊断问题,对外科医师的手术处理有良好的指导作用。

Evaluating diagnosis value of Anomalous Pulmonary Venous Connection (APVC) with MSCT

Objective To evaluate the value of APVC with MSCT angiography. Methods 94 APVC cases collected from 2005 to 2009, which had been diagnosied with MSCT, were analyzed retrospectively, and in which 78 case （male 40 cases,and female 38 cases）were proven in operation to be compared with operation results. Results 78 case of APVC confirmed by operations, 29 cases were TAPVC, in which 7 cases were supracardiac type, 14 cases were intracardiac type, 3 cases were infracardiac type, and 5 cases were mixed type. 49 cases were PAPVC. The accurate diagnosis coincidence rate was 100% compased with surgical diagnosis. Conclusion MSCT has an extremely high accuracy in diagnosis of APVC as a kind of noninvasive technique, which shows intracardiac malformation as well as other complex malformations, and could be a fine guidance for clinical operations.
