核磁共振及诱发电位在多发性硬化中的诊断作用

摘    要:目的:探讨核磁共振(MRI)和诱发电位(EP)在多发性硬化(MS)中的诊断作用。方法:对42例MS同时行头颅和(或)脊髓MRI扫描并对EP结果进行分析。结果:MRI异常为90.48%,表现为长T1长T2大小不等多发性病灶,主要分布于脑室周围、脑干、基底节、半卵圆中心,小部分位于大脑皮质。MRI可发现近1倍亚临床病灶。EP以VEP异常(83.33%)为多见,其次为SEP(64.29%),BAEP较少见(42.86%)。SEP及BAEP尚可提示MS周围神经受损。EP3项检查可明显提高阳性率。MRI发现大脑半球、脑干病灶优于EP,EP发现视神经脱髓鞘及周围神经损害优于MRI。结论:MRI、EP在MS的诊断中有重要作用,结合临床分析则可提高诊断率。EP可以提供周围神经损害的依据。

Abstract:Objective:To investigate the diagnostic value of MRI and EP in multiple sclerosis MS Methods: MRI and EP data were analyzed in 42 patients with MS Results: The abnormal of MRI which was 90 48% Showed small and large and multiple lesions scattering in periventricular areas, brainstem, basal ganglia, cerebral white matter and also in cerebral cortex in small patients The lesions were long T1 and heavyT2 signals MRI could detect two times of subclinical lesions The abnormal of VEP, SEP, BAEP were respectively 83 33%, 66 67% 42 86% SEP and BAEP also could detect the lesions of peripheral nerves Multimode EP could detect more lesions MRI had advantage of EP in detecting the lesions of cerebrum and brainstem EP had advantage of MRI in detecting the lesions of optic and peripheral nerve Conclusion: MRI and EP had great diagnostic value in MS and EP also could detect the lesions of peripheral nerve
