后腹腔镜治疗肾囊肿的临床应用

目的：探讨后腹腔镜肾囊肿去顶术治疗方法、要点和疗效。方法：回顾性分析2008年8月至2011年6月在广东省揭阳市人民医院经腰部腹膜后入路行腹腔镜单纯性肾囊肿去顶术的21例患者临床资料，观察患者的手术时间、术中出血量、术中及术后并发症、住院时间等情况。结果：21例患者手术均获成功，平均手术时间65min。术中出血量20—60mL，平均术中出血量35mL。1例患者出现轻微呼吸性酸中毒，对症处理后缓解。2例患者出现轻微皮下气肿，1-2d后自行缓解。术后住院时间3～5d，平均4．3d。术后随访0．3～2年，无复发患者。结论：后腹腔镜肾囊肿去顶术具有损伤小、出血少、安全有效、并发症少、恢复快、术后住院时间短、治疗彻底等优点，是治疗单纯性肾囊肿的理想术式。

The clinical application of retroperitoneal laparoscopy in treatment of renal cyst

Objective：To investigate the approach, main points and efficacy of retroperitoneal laparoscopy unroofing of renal cyst. Methods： Between August 2008 and June 2011,21 cases undergoing laparoseopic deroofing of simple renal cyst through the retroperitoneum in waist were retrospectively analyzed and observed their surgical time, intraoperatve blood loss, intraoperative and postoperative complications and hospitalization time. Results：The surgery was successfully done in a total of 21 patients （mean surgical time： 65mins）. The intraoperative blood loss was 20 -60 mL （mean：35ml）. One patient was found to have mild respiratory acidosis and gain remission after symptomatic treatment. In addition, two patients were found the presence of subcutaneous emphysema that was spontaneously alleviated at 1 -2 d. The postoperative hospitalization time was 3 -5 days （mean：4.3 d）. In the following-up of 0.3 -2 years,no recurrence was found. Conclusion： Retroperitoneal laparoscopy unroofing of renal cyst has the advantages of less damage, less blood loss, safe, effectiveness, less complications ,rapid recovery, shorter postoperative recovery and thorough treatment, which might be the ideal surgical approach in treatment of simple renal cyst.
