摘    要:目的 应用相干光断层扫描(OCT)动态观察氩激光治疗中心性浆液性脉络膜视网膜病变(CSC)后黄斑水肿吸收情况,探索其在CSC患者预后追踪中的优势.方法 对经荧光素眼底血管造影(FFA)确诊的42例(42只眼)CSC患者进行氩激光光凝治疗,分别于治疗前、治疗后3 d,1、2、4、6周行OCT及FFA检查.结果 OCT适用面广,可清楚的发现神经上皮及色素上皮脱离情况,OCT动态随访观察显示激光光凝治疗3 d时黄斑水肿无明显吸收,1周后,黄斑水肿开始明显吸收,2例患者(4.76%)水肿完全吸收;2周后5例患者(11.90%)水肿完全吸收;4周后17例患者(40.48%)水肿完全吸收;6周后34例(81.95%)水肿完全吸收.结论 激光光凝治疗能缩短CSC病程,OCT为非侵入性,观察效果好,能定量分析黄斑水肿程度,比FFA更加直观、准确.更重要的是,OCT能对预后过程进行动态、精确、灵敏的追踪.

Abstract:Objective Optical coherence tomography(OCT)can record the morphology changes in quantity.In this study we observed the transform of macular edema in patients with CSC who underwent argon laser photocoagulation,to evaluate the effect of OCT in detect and follow up.Methods Forty-two eyes of 42 patients with CSC underwent argon laser photocoagulation and follow up by OCT and FFA at 3day,1week,2week,4week and 6 week.Results The OCT imagines show serous detachment of CSC,including serous neurosensory detachment and serous pigment epithelial detachment.After photocoagulation macular edema is not be absorbed obviously in 3days,it absorbs obviously after 1 week.Two patients'(4.76%)macular edema complete absorption in one week,5(11.90%)in 2 week,17(40.48%)in 4 week and 34(81.95%)in 6 week.Conclusion Argon laser photocoagulation was a effective treatment for CSC.OCT was a good examination for CSC,it was more exact than FFA and could quantitative analysis macular edema,with these advantages OCT was suit for follow up and showed us a clear and accurate process.
