改良Parks手术治疗低位直肠癌29例报告

目的总结低位直肠癌保肛治疗手术经验。方法回顾性分析29例低位直肠癌根治和改良Parks手术吻合及术后随访资料。结果29例中无手术死亡病例，术后吻合口瘘1例，经引流换药等处理后治愈。吻合口狭窄1例，扩肛半年后治愈。29例术后肛门功能良好，1年后每日排便1～2次者29例，良好率100％。术后均采用综合化疗方案化疗，1、3、5年生存率分别为100％（29例）、76．8％（22例）、65．5％（19例）。结论低位直肠癌根治采用改良Parks手术保肛治疗，疗效较好，生活质量较好。

Modified Park's operation on 29 patients with low rectal cancer

Objective To analyze the therapeutic efficacy of modified Park＇s operation for anal sphincter preservation in low rectal cancer. Methods Clinical data of 29 patients undergoing total meso- rectal excision（TME）and modified Park＇s operation for low rectal cancer were analyzed. Results There was no operation death. Postoperative anastomotie leakage occurred in 1 case and was cured by drainage and dressing change. Anaotomatic stricture occurred in 1 case and was cured by dilation for 6 months. The anal sphincter function was recovered in 29 patients postoperatively. After 1 year,29 patients（ 100% ）could defaecate 1 -2 times every day. Postoperative comprehensive chemotherapy was carried out in all cases. The overall 1 - ,3 - and 5 - year survival rate was 100% （29 cases） ,76.8% （22 cases） ,and 65.5% （ 19 cases） ,respectively. Conclusion TME and modified Parkg operation for low rectal cancer is recommendable to preserve anal sphincter function with high therapeutic efficacy.
