弥散加权成像对膝关节腔积液诊断价值的探讨

目的：应用MR SE／EPI DWI序列对在膝关节腔内的滑液进行成像，探讨不同类型积液的弥散特征及弥散成像对关节腔积液的鉴别诊断价值。方法：搜集我院2001年1月－7月间因膝关节疾病出现症状MRI检查的24例患，在不同B值下应用SE－EPI DWI序列对膝关节腔轴位扫描，图像传至Sun工作站，对所有病人的弥散图和ADC图进行兴趣区分析。统计检验采用单因素方差分析和相关性分析。结果：经统计学检验，临床有外伤史的关节腔积液和无外伤史有症状的关节腔积液的ADC值无显性差异；外伤时间的长短对关节腔积液的ADC值并无统计学上的显影响。随弥散强度的增加，无论何种关节腔积液，其信号值均逐渐下降，ADC值降低，不同B值的ADC之间存在相关性。结论：MR弥散加权成像对外伤性和非外伤性关节腔积液的鉴别作用有限，外伤性关节腔积液的弥散特征基本一致，外伤时间不影响关节腔内液体的弥散。

The value of diffusion weighted imaging in the diagnosis of the articular effusion of knee joint

Objective:To study the diffusion characteristics and the value of the diffusion weighted imaging in the differential diagnosis of the articular effusion of knee joint.Methods:SE/EPI DWI with different B value was performed in 25 patients with articular effusion of the knee joint,and the resultant ADC value was calculated at Sun workstation.Results:There was no statistical differences of ADC value between traumatic and non traumatic articular effusion.Time of trauma did not influence ADC value of articular effusion.With increase of diffusion strength,either signal intensity or ADC value of any articular effusion decreased.There was significant correlation between ADC values acquired by different B values.Conclusion:MR diffusion weighted imaging has limited value in the differential diagnosis between traumatic and non traumatic effusion.Time of trauma does not affect the diffusion of the fluid in knee joint.
