慢性肝病126例内分泌功能检测

目的观察慢性肝病(CLD)血清内分泌激素变化。方法采用放射免疫方法测定126例CLD患者的甲状腺激素(FT3、FT4)、性腺激素、胰岛素与C肽、胃泌素、血小板生成素(TPO)和促红细胞生成素(EPO)的血清水平。结果慢性肝炎、肝硬化代偿期与失代偿期的TF3、TF4及促甲状腺激素(TSH)表现基本一致,分别较正常组有明显下降(P<0.001,P<0.005,P<0.005),且与肝脏损害程度成正比;所有CLD患者存在高胃泌素血症;睾酮(T)明显低于正常人(P<0.001),E2明显高于正常人(P<0.005),EPO明显高于正常人(P<0.001),3者均与肝脏损害程度成正相关。TPO明显低于正常人(P<0.05),CLD患者均有空腹高胰岛素血症及高C肽血症。结论CLD常有甲状腺激素和葡萄糖代谢异常、门脉高压性胃病及性功能低下等。

Endocrines function examination in 126 patients with chronic liver disease

Objective To observe the changes of serum endocrinological hormone in patients with chronic liver disease(CLD).Methods The level of thyroid hormones,sex hormones,insulin and C titanium,gastrin,thrombopoietin(TPO)and erythropoietin(EPO) in serum were determined by radioimmunoassay in 126 CLD patients.Results The level of TF3,TF4 and TSH in chronic hepatitis,hepatocirrhosis compensated and hepatocirrhosis decompensated were analogous,lower than those in normal group(P<0.001,P<0.005,P<0.005).All CLD patients were discovered with increased gastrin,insulin and C titanium.Compared with the normal,the testosteron level decreased significantly(P<0.001)and estradiol E2 increased(P<0.005),EPO level increased(P<0.001),there was a positive correlation between the three endocrine hormone and the degree of liver injury,and the TPO was decreased obviously(P<0.05).Conclusion Thyroid hormones and glucose decompensation,portal hypertensive gastropathy,hypogonadism,and hemogram abnormity etc were presented in patients with chronic liver disease.
