经阴道彩色多普勒超声对人流术后阴道流血的诊断价值

目的　探讨经阴道彩色多普勒超声 (TVCDS)对人流术后阴道流血的诊断价值。方法　对 185例人流后阴道流血的患者进行经腹超声 (TAS)、TVCDS检查 ,并与手术及病理结果对照分析。结果　 185例患者中 12 2例为流产不全 ,其残留组织物的二维显像表现为宫腔内不规则高回声及局灶性低回声 ,与宫壁界限清晰 ,CDFI显示其内丰富的彩色血流信号 ,血管呈多种迂曲形态、色彩鲜亮 ;多普勒频谱包络毛糙 ,声音嘈杂 ,呈低阻力型 :RI 0 .4 5± 0 .0 5。结论　 TVCDS对人流不全引起的术后阴道流血具有较高的诊断价值 ,能较好地与宫腔内积血进行鉴别 ,具有临床应用价值

Diagnostic value of transvaginal color Doppler ultrasound in the induced abortion

Objective To evaluate transvaginal color Doppler sonography(TVCDS ) for the diagnosis and differential diagnosis of the vaginal bleeding after induced abortion. Methods A hundred and eighty-five patients who had vaginal bleeding after induced abortion were examined by abdominal ultrasonography and TVCDS,and the results were compared with surgical and pathological findings. Results Among the 185 patients, 122 had vaginal bleeding that was caused by incomplete induced abortion. Two-dimensional ultrasound showed an irregular and uneven high and mixed echo mass in the uterine cavity. Color Doppler ultrasound revealed rich flow areas in the uterine cavity. Blood vessels showed multiple circuitous appearance. Doppler spectrum showed unevenness with lower resistance (RI 0.45).Conclusion TVCDS has a considerable diagnostic value in vaginal bleeding caused by incomplete induced abortion and can be used to differentiate from intrauterine hematocele.[
