摘    要:目的探讨不同钢板治疗老年肱骨近端骨折生物学影响与远期疗效,为合理选择固定材料提供参考。方法选取我院骨科2005年7月至2011年7月收治的老年肱骨近端骨折患者128例,根据患者临床特点与手术方法的不同,分为治疗组和对照组各64例。对照组行AO"T"形钢板内固定术,治疗组行肱骨近端锁定钢板内固定术。结果两组在围术期指标与肩关节疼痛、肌力、关节活动范围与总分方面比较,差异均有统计学意义(P<0.05),治疗组优于对照组。随访6个月,治疗组的优良率明显高于对照组(P<0.01)。结论近端锁定钢板内固定和AO"T"形钢板内固定均是治疗肱骨近端骨折的有效方法,但近端锁定钢板内固定有更好的生物学影响与远期疗效。

Abstract:Objective To investigate the biological effects and long term effect of different steel plates for the treatment of the elderly proximal humerus fractures,to provide references for a reasonable choice of fixed material.Methods 128 elderly patients with proximal humerus fracture admitted in our orthopedic department from July 2005 to July 2011 were selected,and divided into treatment group and control group of 64 cases each,according to the different clinical characteristics and surgical methods.The control group used AO "T" plate fixation,and treatment group were given a proximal humeral locking plate fixation.Results The perioperative indexes andshoulder pain,muscle strength,range of motion,and the total score of the two groups were significantly different(P<0.05),the treatment group better than the control group.Follow-up of 6 months,the treatment group excellent rate was obviously higher than the control group(P<0.01).Conclusion The proximal locking plate fixation and AO "T" plate fixed are effective methods of treatment for proximal humeral fractures,however the proximal locking plate has better biological effects and long-term efficacy.
