正常高值血压与尿微量白蛋白的关系

摘    要:目的探讨尿微量白蛋白含量与血压水平的相关性。方法根据《2004年中国高血压防治指南》选择正常高值血压者(≥120/80mmHg)109例及正常血压者(〈120/80m/nHg)49例,测量血压,计算体重指数(BMI),测定空腹血糖(VBS)、血清总胆固醇(CHO)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL—C)、低密度脂蛋白胆固醇(LDL—C)、尿微量白蛋白(EusA法)等,并做比较。结果正常高值血压组与正常血压组相比,尿微量白蛋白含量升高(3.08±1.58)mg/L比(3.86±1.7)mg/L,P〈0.01],CHO、TG、HDL—C和LDL—C两组差异无统计学意义(P〉0.05)。Spearman相关分析显示,尿微量白蛋白与血压、血糖呈正相关,相关系数收缩压r=0.177,P〈0.05;舒张压r=0.252,P=0.001;空腹血糖r=0.279,P〈0.01。校正年龄、性别、血脂、体重指数偏相关分析表明,尿微量白蛋白含量与血压、血糖水平仍呈正相关,相关系数收缩压r=0.156,P=0.055;舒张压r=0.233,P〈0.05;空腹血糖r=0.251,P〈0.05。结论①正常高值血压组尿微量自蛋白水平较高,血糖、体重指数有异常变化,但血脂无异常变化。②尿微量白蛋白发生与舒张压、血糖关系更密切。

Abstract:Objective This study was performed to investigate the relationship between blood pressure and microalbuminuria. Methods According to 2004 guideline for the preventing and treating hypertension of china, 109 high-normal blood pressure individuals (≥120 or 80 mm Hg) and 49 normal blood pressure individuals (〈120/80 mm Hg) were selected from the people who are fit for the critia. The blood pressure, body mass index ( BMI ), fasting blood glucose (FBS), total cholesterol( CHO ), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) ,low-density lipoprotein cholesterol (LDL-C), Urine albumin excretion measured by ELISA were measured and compared between the normal blood pressure and high-normal blood pressure groups. Results Compared with normal blood pressure. The microalbuminuria is higher in the group of high-normal blood pressure (3.08±1.58 )mg/L vs (3.86±1.7 )mg/L,P〈0.01 ], however, CHO, TG, HDL-C, LDL-C, was not found difference in the two groups (P〉0.05). Spearman correlation analysis showed microalbuminuria correlated negatively with blood pressure and fasting blood glucose SBP r=O.177,P〈O.O5,DBP r=O.252,P=O.O01 ,FBS r=O.279,P〈O.01. Adjusted age, sex, blood lipoprotein and BMI we received the same result, microalbuminuria levels were positively correlated with diastolic blood pressure and fasting blood glucose,SBP r=O.156,P=O.O55,DBP r=0.233,P〈0.05,FBS r= 0.251 ,P〈0.05. Conclusion (1)Compared with group of normal blood pressure,high-normal blood pressure group appears to be associated with higher microalbuminuria,fasting blood glucose, BMI but not CHO,TG, HDL-C,LDL-C. (2)The major factor influencing microalbuminuria(MAU ) was diastolic blood pressure and fasting blood glucose.
