人工全髋置换术和人工股骨头置换术治疗老年股骨颈骨折的疗效比较

目的比较人工全髋置换术与人工股骨头置换术治疗老年股骨颈骨折的疗效。方法采用回顾性分析的方法,收集2004年1月～2006年1月我院收治的278例65岁以上老年股骨颈骨折病例,其中156例采用人工全髋关置换术,122例采用人工股骨头置换术,评价两种手术方式的时间和术中失血量,并采用Harris评分比较这两种手术方式对老年人股骨颈骨折的治疗效果。结果人工股骨头置换术较人工髋关节置换术手术时间短、失血量少,2组病人经过平均3年的随访后人工股骨头置换术的远期并发症和再手术率高,而人工全髋关置换术髋关节功能优良率较高。结论人工股骨头置换术适和于高龄全身状况较差的患者,而人工全髋置换术适于身体状况较好的高龄股骨颈骨折患者。

TO COMPARE THE EFFICACY OF ARTIFICIAL ENTIRE HIP REPLACEMENT OPERATION AND ARTIFICIAL THIGHBONE HEAD REPLACEMENT OPERATION IN THE AGED FEMORAL NECK FRACTURE

Objective To compare the etfficacy on the treatment of aged femoral neck fracture by artificial entire hip replacement operation and artificial thighbone head replace operation. Methods We enrolled 278 patients who were older than 65 years from January 2004 to January 2006. 156 patients were undertook artificial entire hip replacement operation and another 122 were undertook artificial thighbone head replacement operation. We applied a retrospective analysis to evaluate the operative time and the loss of blood and to compare the long term efficacy of these two procedures by Harris evaluation system. Results The artificial thighbone head replacement operation cost little time and loss less blood than the artificial entire hip replacement operation. After the average 3 years follow up, the long term complications and re -operation rates were higher in artificial entire hip replacement operation group than the artificial thighbone head replacement operation group and the artificial thighbone head replacement operation group had a higher score by Harris evaluation system. Conclusion Artificial entire hip replacement operation was suitable to the aged poor condition patients whereas artificial thighbone head replacement operation was suitable to the aged well condition thigh neck fracture patients. thighbone head replacement operation
