支气管镜下氩等离子体凝固在气道疾病中的应用

摘    要:目的探讨局部麻醉下氩等离子体凝固在气道疾病中应用的有效性及安全性。方法局部麻醉下经电子支气管镜应用氩等离子体凝固(APC)300型氩气刀治疗77例患者的气道阻塞性疾病。结果77例伴有92处中心气道阻塞的患者接受APC300型氩气刀治疗167次,24例恶性肿瘤有效率为84.0%,53例良性肿瘤及支气管结核有效率为94.3%;疗效还与病变位置有关,位于气管的病变有效率为100%,主支气管及右中间段支气管的病变疗效为95.3%,而叶支气管的病变疗效为82.4%。结论在局部麻醉下采用APC300型氩气刀可以有效、安全地治疗气道疾病,特别是大气道的良性病变。

Abstract:Objective To evaluate the effect of argon plasma coagulation for resecting central airway obstructive lesions via the flexible bronchoscope under local anesthesia.Methods Endobronchial therapy with argon plasma coagulation(APC) 300 made by ERBE in Gemany was performed via flexible bronchoscopy under local anesthesia for patients with central airway obstructive lesions.Results Between Jan 2008 and Sep 2011,77 patients with 92 lesions recieved 167 APC procedures.Efficacy rates were 84.0% of 24 cases with malignant tumor,94.3% of 53 cases with tuberculosis,benign tumor,efficacy rates were 100% of lesions in tracheal,95.3% of lesions in main stem bronchi and bronchus intermedius,82.4% of lesions in lobe bronchi.Conclusion APC is an effective and safe method to resect airway obstructive lesions,especially those benign lesions in big airway.
