大肠癌患者骨桥蛋白检测的临床意义

目的探讨骨桥蛋白（OPN）作为大肠癌肿瘤标志物检测的临床意义。方法 2007年1月至2013年12月收治212例大肠癌患者,应用酶联免疫吸附试验（ELISA）检测患者手术治疗前后血清0PN的质量浓度,用免疫组织化学法测定肿瘤组织OPN的表达。对比血清OPN在手术前后的变化,并与对照组（正常健康志愿者）60例比较;分析血清OPN与大肠癌组织OPN的表达、临床特征和病理特征的相关性。结果血清OPN在大肠癌中检测的敏感性和特异性分别为70.3%（149/212）和83.5%（177/212）。大肠癌血清OPN水平依肿瘤组织OPN表达阳性率由低到高的顺序递升,差异有统计学意义（P〈0.05）;并与临床分期、脉管内癌栓、淋巴结和器官转移相关（P〈0.05或P〈0.01）,特别与肝转移密切相关（P〈0.01）,与肿瘤大小及分化程度无相关性（P均〉0.05）。治疗前后OPN的质量浓度差异有统计学意义（P〈0.01）。结论 OPN是一种较理想的大肠癌肿瘤标志物。

Clinical significance of osteopontin detecting in patients with colorectal cancer

Objective To evaluate the clinical significance of osteopontin （OPN） as a tumor marker of colorectal cancer. Methods A total of 212 patients with colorectal cancer admitted in Jiangsu Cancer Hospital between January 2007 and December 2013 were selected,and sixty healthy subjects were served as control group. The serum OPN mass concentration of pre- and post-operation in colorectal cancer patients and in control group were detected by enzyme linked immunosorbent assay（ELISA） method. The OPN expression in cancer tissues was assayed by immunohistochemical method. The relationship between serum OPN level and OPN expression in eolorectal cancer tissues and the relationship between clinical features and pathological features were analyzed. Results The sensitivity and specificity of serum OPN detecting in colorectal cancer patients were 70.3 % （ 149/212 ） and 83.5% （ 177/212 ） , respectively. Serum OPN levels of colorectal cancer patients presented ascending progressively along with the sequence from low to high of OPN positive expression rates in cancer tissues,and the difference had statistical significance （ P 〈 0.05 ）. The serum OPN levels were correlated with clinical stage,intravaseular cancer embolus,the metastases of lymph node and the metastases of organ（ P 〈 0.05 or P 〈 0.01 ）, especially with hepatic metastasis（ P 〈 0.01 ）, while not correlated with the size and differentiation degree of tumor（ all P 〉 0.05）. There was significant difference between pre- and post-operative OPN mass concentration（ P 〈 0.01 ）. Conclusion OPN is an ideal tumor marker for colorectal cancer.
