妊娠期糖尿病胰岛素治疗的临床分析

目的研究妊娠期糖尿病使用胰岛素治疗对妊娠结局的影响。方法回顾分析2010年6月至2011年6月成都市妇女儿童中心医院收治的147例妊娠期糖尿病（gestational diabetes mellitus，GDM）患者资料。随机选择同期住院分娩正常孕妇150例为对照1组；无需胰岛素治疗的124例GDM为对照2组；需胰岛素治疗的23例为研究组。比较3组孕妇一般情况、妊娠结局及母婴并发症等情况。结果3组孕妇的母婴并发症发生率比较无统计学意义（P〉0．05）；剖宫产率研究组和对照2组明显多于对照1组，差异有统计学意义（P〈0．05）；手术指征比较，因社会因素导致的剖宫产研究组和对照2组明显多于对照1组，差异有统计学意义（P〈0．05）。结论在饮食运动无效后及时合理的使用胰岛素能有效降低母婴并发症。

Clinical analysis of insulin therapy for gestational diabetes mellitus

Objective To study the effects of insulin therapy on the pregnancy outcome in gestational diabetes mellitus（GDM） women. Methods The data of 147 eases with GDM from June 2010 to June 2011 treated in Chengdu Central Hospital of Women and Children were analyzed retrospectively. Another 150 normal cases without GDM who delivered in the same hospital were selected randomly as Control Group 1 ; 124 eases with GMD who did not need insulin were collected for the control group 2 and the other 23 patients with GDM who needed insulin therapy were selected to participate in study group. The differences on gravid general situation, pregnancy outcomes and maternal and neonatal complications among three groups were observed. Results The difference of maternal and neonatal complications were no significant （P 〉 0. 05 ） among the three groups. The cesarean section rates were significantly higher in control group 1 and study group than that in control group 2 and the data were statistically significant, which was caused by more social factors cesarean section required by pregnancy women with GDM. Conclusion Timely and proper use of insulin would effectively decrease maternal and neonatal complications while the diet and exercise therapy was null and void.
