摘    要:目的 了解近3年来大肠埃希菌的临床分布及其耐药性变化,为临床治疗提供依据. 方法 回顾性分析邵阳市第一人民医院2009-2011年住院患者细菌培养检出的大肠埃希菌及产ESBLs菌株分布情况及耐药率. 结果 各种常用抗生素的耐药率从2009-2011年有不同程度的增长,特别是喹诺酮类抗生素耐药率上升明显,环丙沙星和左氧氟沙星的耐药率分别上升到54.4%~87.2%和52.6% ~84.4%,产ESBLs菌株的检出率也明显增加,为42.5%. 结论 临床常用抗生素耐药率近3年来都有不同程度的增长,特别是喹诺酮类抗生素,而产ESBLs菌株耐药率更为严重,应根据其耐药特点及变化趋势,控制播散耐药菌株的流行.

Abstract:Objective To investigate the clinical distribution and drug resistance of Escherichia coli in the past three years, and to provide a basis for clinical treatment. Methods We retrospectively analyzed the distribution and drug resistance rates of Escherichia coli and ESBLs- producing strains isolated from specimens of patients hospitalized in the First People's Hos- pital of-Shaoyang from 2009 to 2011. Results During the period of 2009 - 2011, the resistance rates of Escherichia coli to commonly used antibiotics were increased in different degrees, especially the significantly increased resistance rates to fluoroquin- olone antibiotics. The resistance rates of Escherichia coll to ciprofloxacin and levofloxacin increased from 54.4 % and 52.6 % to 87.2 % and 84.4 %, respectively. The detection rate of ESBLs- producing strains was significantly increased, reached 42.5 %. Conclusions In recent three years, the resistance rates to commonly used antibiotics have been increasing in different de- grees, especially to fluoroquinolone antibiotics; moreover, the drug resistance rate of ESBLs- producing strains is higher. More attention should be paid to the characteristics of drug resistance and changing trend of Escherichia coli and ESBLs- producing strains so as to control the prevalence of drug - resistant strains.
