胰岛素泵治疗Ⅰ型糖尿病酮症酸中毒临床观察

目的 比较胰岛素泵持续皮下胰岛素输注(CSⅡ)法与传统小剂量胰岛素静点法治疗糖尿病酮症酸中毒(DKA)的短期疗效.方法 DKA病人56例,随机分为CSⅡ组和对照组,CSⅡ组予胰岛素泵持续皮下输注诺和灵R,对照组予小剂量诺和灵R持续静点.结果 CSⅡ组酮体转阴快(P＜0.05),两组达到相同的血糖水平,CSⅡ组所需的治疗时间、胰岛素用量以及低血糖发生率明显减少(P＜0.05).结论 胰岛素泵治疗DKA酮体转阴快,更快、更有效的控制高血糖,减少低血糖发生.

Clinical observation on insulin pump infusion in the treatment of diabetic ketoacidosis

Objective To observe the short term therapeutic effects of continuous subgutaneous insulin infusion (CSII) in the treatment of diabetic ketoacidosis ( DKA) ,and compare with continuous venous insulin infusion. Methods 56 Patients with DKA were randomly divided into CSII group and control group. In CSII group, the Novolin R was subcutaneously infused using insulin pump continuously. In control group, the Novolin R was continuous venous infusion. Results The disapenrena of ketonuria of the CSII group was more rapid than that in the control group (P <0. 05 ). Hyperglycemia of the CSII group was depressed, and decreasing degree was similar to that of the control group. However, therapeutic time, insulin dosage and insulin induced hypoglycemia reaction incidence in the CSII gloup were obviously lower than that in the control group (P <0. 05). Conclusions DKA treatment through using insulin pump infusion could make more rapid disappear ketonuria, and could be more effective and more rapid control over the blood gloucose level, and could reduce the incidence of hypoglycemia reaction.
